An inmate in "Enhanced Restraints Status" shall be handcuffed behind his back utilizing a waist chain and leg irons prior to removal from his cell and shall remain so restrained. During all movement outside the housing area, facility, and during all program activities, except under the following circumstances:

a) Medical emergencies with staff approval
b) Family visitation times
c) Meals in designated areas
d) Recreational time under supervision
