Mrs Freeman is a 40-year-old client with schizophrenia and bipolar disorder. She is unable to work and spends her time at home with visits from her carers to provide support services.
She suffers from hallucinations and hears voices which cause her to become anxious and extremely emotional. The schizophrenia also causes Mrs Freeman to assault others, both verbally and physically. The bipolar disorder has significant effects on Mrs Freeman’s emotions, causing feelings of despair and worthlessness. Recently, Mrs Freeman has become increasingly withdrawn and has stopped participating in her regular social events.
Mrs Freeman’s carers give her separate medications to treat her schizophrenia and bipolar disorder. These medications do alleviate the symptoms for some time. However, they have negative side effects, including trembling, drowsiness, nausea, and vomiting. Her friends and family members are struggling to cope and maintain the support that they have provided over the years.
Organisational reporting policies and procedures
Reporting policies outline that reports must be fair, impartial and accurate. They are supportive of the client’s needs.
Reporting procedures include providing a report date and author. Reports must be in short and simple sentences to ensure details are understood. Information must be sequenced in order of events, and there should be a report conclusion that identifies any next steps.
