Patient presents for her usual headache. She has recurrent headaches, approximately every month or so. She is seeing a neurologist for further evaluation. Headache began as usual with some nausea and vomiting. Has already taken Compazine without any relief. She appears uncomfortable. VS normal, has difficult opening eyes due to photophobia. Lungs and heart and normal. Administered 75 mg of Demerol and 75 mg of Vistaril IM with some relief. She will return if symptoms do not improve. Assign code(s) for facility services.
