Indication for Procedure: This 43 y/o woman has a history of progressive left hearing loss and was found on imaging to have a cerebellopontine angle and internal auditory canal tumor. For these reasons, the above-named procedures were indicated. Prior to surgery, the patient understood the risks of the operation included facial nerve injury, including paralysis, cerebrospinal fluid leak, and intracranial complications (meningitis, brain abscess, stroke and death), numbness of the ear, ringing in the ear, dizziness and possible need for further surgery. She understood she would lose all remaining hearing in the operated ear. Medical coding
