Mr. Jerry Smith is a caucasian 54 year old male born on December 12. He
presents to the office to complete his annual wellness exam. He had blood
drawn last week prior to this appointment. His current medications include
losartan 50mg PO QD. Mr. Smith reports a family history of HTN,
hyperlipidemia and CAD, his father passed away at the age of 65 due to an MI.
His mother has type 2 Diabetes. Mr. Smith works for a local electrical company
and is married. Drinks alcohol socially and was a heavy smoker when he was
younger but currently only smokes 1-2 cigarettes a week depending on stress
with work.

Weight: 236lbs. BMI: 31, Appears well, Cardiac regular rhythm and rate, no
murmur, Resp: lung sounds clear to all, no rales, rhonchi or wheezing,
