First find the perimeter. The formula for that is P=2L+2w
The perimeter is 32.

SA=32*3+2*12              (I think its 12 for base)
SA=96+24
SA= 120 cm2

The equation to find the surface area of a prism is:
h * P + 2 * b
H = Height
P = Perimeter
B = Area of Base

To find the perimeter you have to do this equation:
2l + 2w 
L = length
W = width

2 * 12 + 2 * 4 = 32
P = 32

Then finding the area of base using this equation:
L * W

12 * 4 = 48
B = 48

Now, put it all together
H = 3
P = 32
B = 48

3 * 32 + 2 * 48 = 192 cm^2
