Think of slope as rise over run. How much does the line go up, every time it goes over. Simply find two points that are nice and even on the graph. Then count how much it goes up between these two point, and how much it goes over. Then write as rise over run. If the graph goes up to the right, then it’s positive. If it goes down to the right, then it’s negative

Y intercept is where the graph crosses the y axis. The intercept will be (0, y). It’s just the crossing point

For the equation, just put the last two things I went over together. I remember by saying y = move x plus begin. This stands for y=Mx+b
The move is the slope. Put in the fractional slope, which will be either positive or negative. Then write x. Then do the y intercept. Which is how far up or down on the y axis it starts.
For example. The equation for the first one is y=3x+6. It rises 6 for every time it goes over 12, which is simplified to 3/1. And it starts at y=6
